Photograph and Publicity Release Form
I, the undersigned, hereby grant Azevedo Water Polo Inc. (AWP) and its Organizers (as defined below) permission to use my name, likeness, image, voice, and/or appearance as such may be embodied in any photographs, video recordings, audiotapes, digital images, or similar media captured or created on behalf of AWP activities. I understand and agree that AWP and its Organizers have full ownership of such media, including all copyright rights, and may use them for any purpose consistent with the mission and goals of AWP.
These uses include, but are not limited to, illustrations, bulletins, exhibitions, videotapes, reprints, reproductions, publications, advertisements, and any promotional or educational materials, whether in print, digital, or electronic formats. This includes distribution across all media now known or later developed, including the Internet.
I acknowledge that I will not receive any compensation or other benefits for the use of such media and hereby release AWP and its agents and assigns from any and all claims, demands, and causes of action that may arise from or be in any way connected with the use of my likeness, image, voice, or appearance as described above.
Definition of Organizers
For the purposes of this release, “Organizers” refers to Azevedo Water Polo Inc., Sara Azevedo, Tony Azevedo, and their respective directors, officers, members, employees, partners, coaches, volunteers, officials, participants, clubs, agents, sponsors, and owners/operators of facilities. This also includes, but is not limited to, 6-8 Sports, Azevedo Aquatics, and USA Water Polo.
I have read and fully understand the contents of this consent and release form. I hereby give my consent to Azevedo Water Polo Inc. and its Organizers to use my name, likeness, and image to promote AWP programs, its partners, and their associated activities.
Azevedo Water Polo Inc. (AWP, AzeItalia) Disclaimer
This document serves as a formal disclaimer for all business conducted by Azevedo Water Polo Inc. (AWP, AzeItalia), which organizes water polo clinics, tournaments, and other events worldwide. The company also arranges team and individual water polo training trips within the United States and internationally.
Liability and Responsibility
Please be advised that Azevedo Water Polo Inc., and by extension Sara Azevedo, Tony Azevedo, and Cassandra Azevedo Maggiolo, are not responsible for any issues, problems, or mishaps that may arise during any phase of an AWP event or training trip. This includes, but is not limited to, logistical issues, injuries, travel delays, or any other unforeseen circumstances that may occur.
It is the responsibility of athletes, parents, volunteers, chaperones, or any other individuals involved to ensure they possess valid local or international health insurance, along with any other necessary insurance, for the duration of their participation in the event or training trip. Azevedo Water Polo Inc. will not cover any costs, expenses, or damages related to such occurrences.
Release and Indemnification
I, the undersigned, hereby acknowledge that I am fully aware of the risks and hazards associated with, or related to, travel, water polo, intense training, athletic activities, and residing in foreign countries.
On behalf of myself, my children, wards, heirs, assigns, personal representatives, and next of kin (collectively, the “Releasees”), I understand and agree to accept all risks and hazards and take full responsibility for any injury or loss that I or my minor child/ward may incur while participating in a training program, traveling to/from, or during the event.
By signing this agreement, I, on behalf of myself and the Releasees, hereby release the Organizers from any claims, demands, actions, expenses, costs, damages, and causes of action, whether arising in law or equity, in respect of death, injury, loss, or damage to person or property, HOWSOEVER CAUSED, arising from or related to my or my child/ward’s participation in any event, training, or competition organized by AWP or its partners. This release applies whether the cause was due to negligence or otherwise.

Acknowledgment
By signing this document, I acknowledge that I have carefully read and fully understand the terms and conditions set forth in this disclaimer and agree to be bound by them.
